HWC Programs - Professional Sanitation Seminar for Summer Festival Food Vendors Online Reservation Form

In Partnership With The "City of Chicago" & The Chicago Department of Public Health

Dates: March 25 - September 11, 2008

Purpose:

This online form will allow interested students to make a reservation to participate in a three-hour sanitation seminar for preparing food vendors to operate in compliance with applicable rules and guidelines for safe outdoor food handling. Classes are offered on Tuesday afternoon and Thursday evening. All classes are taught by certified instructors.

Location:

All classes are held at Harold Washington College, 30 East Lake Street, Room 810.

Cost:

$30.00 includes all handouts and certificate. "Payment by money order only!" Credit card payments and cash can be accepted by the College Cashier when registering in person at the college. Please make your $30 payment by money order payable to Harold Washington College.

Lake Street, Chicago IL 60601. Please note special instructions about filling out and returning mail-in registration form. Please note that no walk-in registrations will be accepted on the day of a class. Classes are conducted in English unless otherwise indicated.

For information call: (312) 553-5802/ 5803 / 5807. You must be on time and stay for the entire three hour seminar. You may only attend class on the chosen date and time. Certificates will be issued at the end of the 3-hour seminar. The college reserves the right to cancel any class in which enrollment is insufficient.
